This website collects no PHI
The forms on this site deliberately collect business contact details only. Please do not submit patient names, identifiers, claim details or any clinical information through them.
How PHI is handled in an engagement
Protected health information is exchanged only after a Business Associate Agreement is executed, through secure channels agreed with your practice, and accessed on a minimum-necessary basis.
Access control
Access is granted by role and reviewed on a defined schedule. Joiners, movers and leavers are handled through a documented process.
Audit trails
System access and changes are recorded against an individual user.
Certifications
We publish a certification or attestation only when it has actually been obtained. Where this page is silent, assume no such certification is claimed.
